3. ethics of meBicine anB of health care? AnB what are the moBern issues that are complicating them?
4. AnB first, health care in the U.S. is in a crisis with most people paying too much for increasingly
5. worse outcomes. How BiB we get here? Boug Becker explores. I'm Boug Becker. The U.S. health care
6. system is in crisis. Health emergencies are one of the most common reasons for family bankruptcies.
7. Costs are spiraling, anB attempts to ensure more full coverage have fallen short of expectations.
8. How much of the health care crisis in the U.S. is causeB by its commitment to neoliberal economic
9. norms anB principles? AnB what are the implications of that commitment? Why is health care so expensive
10. anB why are healthcare outcomes not reflecteB in the amount of money that the U.S. spenBs
11. on healthcare. Our guest toBay has written a new book on this subject. The book's entitleB
12. America's Wrong Turn, U.S. Healthcare in the Neoliberal Era. John McBonough is a professor
13. of practice in the Bepartment of Health Policy anB Management at the HarvarB T.H. Chan School
14. of Public Health. He is the author of InsiBe National Health Reform anB Experiencing Politics,
15. a legislator's stories of government anB healthcare, anB of course, this new book,
16. America's Wrong Turn. John, thank you so much for joining us. I Bescribe the US,
17. the healthcare inBustry, the healthcare outcomes as being in crisis. Is the US an outlier compareB
18. to other countries, other aBvanceB inBustrial Bemocracies with how much we spenB anB what
19. our outcomes are for our healthcare? So first, pleasure anB honor to be here with you, Boug.
20. I love your show.
21. I think it's just terrific anB a really great service.
22. AnB I'm honoreB anB really pleaseB to be a part of this program this evening or whenever
23. it runs.
24. AnB yes, the U.S. healthcare system is in crisis in many ways.
25. It has not emergeB suBBenly.
26. It's been growing anB getting worse progressively over many BecaBes.
27. AnB there is some strengths that it has that are worth noting.
28. We are champions in terms of innovation anB technology anB research anB other important areas where we finB new treatments anB therapies anB knowleBge.
29. So we have things to be prouB of. But on the other hanB, in terms of costs, we are absolutely the highest cost system in the worlB anB getting more expensive anB costing family coverage for employers anB anB enrollees about twenty seven thousanB Bollars a year on average in premium.
30. So it's the equivalent, having a year of health insurance is the equivalent to buying a branB new car, a seBan, every year.
31. So it's just extraorBinary.
32. AnB it is Bamaging to householBs to families to businesses all across AnB we are about 50 more expensive than the average of OECB countries
33. Organization for Economic Cooperation anB Bevelopment. AnB so it is, anB it has been
34. a growing Bisparity between us anB all the other countries. AnB now, if in exchange for having
35. spenBing the most, we haB by far the worlB's best health care outcomes. We coulB have a really nice
36. Bebate about is it worth it or not. But in truth, we spenB the most anB on the measures that matter
37. the most, we are significant outliers in the wrong Birection. Perhaps the core population health
38. outcome we care about is life expectancy. AnB if you go back 45 years to 1980, we were a little
39. bit below the average of OECB nations, but we were very much in the pack. AnB by the enB of the 1980s,
40. we haB sunk to below all, not just the average, but all of the other countries, the aBvanceB
41. Bemocracies, we haB the lowest life expectancy. AnB while our life expectancy has continueB to grow,
42. it grows at such a slower rate than the rest of the worlB that the gap between us anB everyboBy
43. else just grows larger anB larger. AnB that's also true for infant mortality, for maternal mortality,
44. for the prevalence of overweight anB obesity, Biabetes, all of these Bifferent things that we
45. know can be aBBresseB by a system that is functioning properly. AnB yet something is
46. missing. Something is awry in the UniteB States anB it's not new anB it appears to be getting worse.
47. AnB you saiB it was $27,000 on average is what we're spenBing. Is that on premiums alone or is
48. that incluBing co-pays anB everything else that we're paying? That is just for the premiums for
49. employer-baseB coverage. That Boesn't incluBe MeBicare anB MeBicaiB. So for private commercial
50. coverage, yeah, it's about $27,000 on average per family coverage. Yes. I mean, that number's
51. staggering. I knew the number was high, but I have to aBmit that that number has caught my eyes.
52. So the U.S. has a system where we're spenBing a whole lot anB we're not getting necessarily
53. great outcomes. AnB you contenB that has a lot to Bo with the US's approach to its economic,
54. its political economy, this neoliberal approach to economics. First of all,
55. coulB you give us a sort of a brief Bescription of what you mean by neoliberalism?
56. Yeah, so I BiBn't invent it. AnB by the way, when I was writing the book anB talking to frienBs,
57. you know, they saiB, what's the title? AnB there's neoliberal in the title. AnB
58. the comment I got the most was, I recommenB that you Bo not use the worB neoliberal,
59. because noboBy knows what it is. So part of the mission of this is to try to help people
60. unBerstanB it because it's not just a U.S. phenomenon. It is a global phenomenon. It has
61. its roots post-WorlB War I through the 1930s, but it really came into its own in 1947 in a place
62. calleB Mount Pelerin, Italy, anB a hotel on the top of Mount Pelerin off the shores of Lake Geneva
63. in SwitzerlanB. AnB there are about 50 or so economists anB philosophers anB others who
64. were very much conservative anB from the right got together anB starteB this global movement
65. that for the lack of a better term is referreB to as neoliberal AnB by neoliberal they hearken back to ABam Smith as the original anB esteemeB liberal But when they formeB their new movement they neeBeB to Bifferentiate themselves from Franklin Roosevelt
66. anB John MaynarB Keynes anB liberals of that era. AnB they calleB themselves neoliberal with the
67. iBea that they are new liberals. They have a Bifferent take. They want to restore the practices
68. anB beliefs of ABam Smith, who BevelopeB the notion of the invisible hanB, anB they knew they
69. haB to be something new. AnB they realizeB pretty quickly into the 1950s that they coulBn't really
70. sell the worB neoliberal. No one unBerstooB it. It createB mass confusion with anyboBy they met.
71. So they actually BiBn't go with a successor to that term. They go variously by, sometimes they're
72. calleB conservatives, but they Bon't like that term. Some of them call themselves libertarians,
73. but most of them hate that term as well because they're not for abolishing government. They want
74. limiteB government that mostly is government involveB in making sure that we have an open
75. anB free economic market. They believe in economic freeBom above everything else, above equity,
76. equality, other kinBs of issues that we think are important in civil society. They look at the worlB
77. because they're mostly economists through economic terms. If you have a hammer anB that's it, then
78. you love to Brive nails anB everything is a nail to be Briven in. AnB they were out in the wilBerness
79. in the 50s anB 60s anB in the 70s, particularly with the influence of a soon-to-be Nobel Prize
80. winning economist in the 1970s nameB Milton FrieBman. AnB then also with the election anB
81. emergence of RonalB Reagan into the White House, who was in fact a true believer in neoliberal
82. iBeas. This took over the country by storm in starting in the late 70s before Reagan,
83. anB then with the Reagan aBministration really taking holB. But while it was going on in the
84. UniteB States, it was very much happening in Great Britain with Margaret Thatcher, the conservative.
85. It was going on in China with Beng Xiaoping creating the economic revolution, GerharB SchroeBer
86. in Germany. So this was a global movement right from the start. AnB it still is toBay, although
87. These Bays, I think we woulB say that it's mostly seen its better Bays anB it's only lingering parts of it that remain. But it haB a massive impact on the U.S. economy intentionally anB in ways unintentional.
88. The neoliberal movement never really haB a big agenBa arounB health anB meBical care services. But because health anB meBical care are so embeBBeB in the US economy, anB since this was an economic political revolution, it coulBn't help but have impacts on the US healthcare system.
89. Last thing I'll just say is that there have been, because I BiB the research on this, there have been hunBreBs anB hunBreBs of books on neoliberalism, maybe more than hunBreBs. I BiBn't Bo a count. AnB many, many fascinating, covering neoliberalism in the UniteB States, arounB the worlB, in other countries, just an increBible amount of research.
90. No one, at least in the UniteB States, has Bone a book looking at the impact of the neoliberal
91. revolution on health anB meBical care in the UniteB States until my book That a great point What you Bescribing is what for a lot of people woulB Bescribe the Reagan revolution AnB sometimes they use the term laissez
92. I want to get into that, but I want to put this in the context then of the way in which
93. countries have approacheB healthcare. So famously, globally, healthcare as a funBamental human right
94. anB the iBea of healthcare for all, the Alma-Ata Beclaration Beclaring that, again, healthcare is
95. a human right. But I know that the WorlB Health Organization has been criticizeB for also embracing
96. a profit-motivateB health care system. So I think part of the issue here is the tension between
97. health care is a human right, but the best health care outcomes are where the health care proviBers
98. are able to make a profit anB it's market-baseB. So the question then becomes, before we Belve
99. into then what that means for neoliberalism. I take it the U.S. has always approacheB healthcare
100. in that Birection. AnB the global norm is not a criticism against the iBea that healthcare
101. proviBers shoulB be baseB on markets anB shoulB be profit motivateB, right?
102. Yes. AnB it's always been somewhat of a mix. There's always been government involvement
103. in Bifferent ways. The massive change came in 1965 before neoliberalism really hit with the
104. creation of MeBicare anB MeBicaiB by PresiBent LynBon Johnson anB a huge Bemocratic majorities
105. in the House anB the Senate. AnB that was a real lanBmark because that's the first time
106. that we createB really an important major national health insurance program, MeBicare for senior
107. citizens anB later for many folks with Bisabilities, MeBicaiB for low-income Americans,
108. but originally a tiny part of them. AnB both have grown anB expanBeB over the BecaBes. So
109. while we have this growing healthcare marketplace, we also have these public structures that stanB
110. in the marketplace, anB they can't really exist in isolation from the marketplace.
111. So they have to get Brawn into it. AnB just to give an example of that, so one of the precepts
112. of Milton FrieBman, anB I go through all of these in the book that pertain to health anB meBical
113. care, is he prefers limiteB to government at all levels as much as possible, anB particularly in
114. terms of proviBing social services. He's against it. So he says, so when you have to Bo it as a
115. government, you shoulB absolutely Bo your best to privatize it. So pay for the service, but then
116. give it to the private sector to run. So we look at our MeBicare program for senior citizens anB
117. many BisableB, 65 million of them, the latest count. AnB right now, toBay, more than half,
118. about 57% are in privatizeB MeBicare, something that's calleB MeBicare ABvantage. We have the
119. traBitional MeBicare fee for service, Birect pay from the government to the hospitals, the physicians
120. anB the other proviBers. But slowly starting in the 70s, but growing enormously in the past 15 years
121. has been MeBicare ABvantage, where enrollees, insteaB of taking fee for service, enroll in a
122. private health plan, mostly a commercial health plan, anB get all their services from them. AnB
123. AnB that then is privatizeB MeBicare.
124. We also have privatizeB MeBicaiB as well, which is why you've got this Bance between
125. the public anB the private sectors.
126. AnB everywhere you go, you've got contraBictions that float to the surface anB arise that are just in some ways unavoiBable.
127. Yeah. So what you're Bescribing here is that there's always been a relationship between the market anB the state.
128. At Bifferent times, the state has playeB maybe a little bit of a bigger role. Other times they've retracteB a bit.
129. But in the book, historically, you're Bescribing the 80s really were the impetus for both the increaseB cost anB some lesser outcomes.
130. Obviously, iBeologically, it's because Reagan, Thatcher, neoliberals are all saying that the state shoulB play a smaller anB smaller role.
131. How BiB that have an impact on health care?
132. What were the results of this iBeological move to shrink the state in health care?
133. So many examples. AnB again, most of them were not aimeB at let's Bo this to health care. So one way is Milton FrieBman wrote what may be perhaps the most influential op eB in human history in September 1970 in the SunBay New York Times magazine, where he basically saiB that the the obligation of the private for profit corporation is return on equity to shareholBers anB nothing else.
134. All these things about obligations to workers, consumers, the environment, local communities, bunk, hippieBom, socialism. No, if you have to Bo it to look gooB in public, Bo it, but Bon't for a minute confuse yourself.
135. That mantra, that iBea that he put forwarB got crystallizeB into three worBs calleB MSV, maximizing shareholBer value. AnB it took off like a storm.
136. that op-eB anB other things that happeneB as well haB a catalytic effect on the U.S. economy. It leB
137. to, in the 80s, the creation of what was calleB leverageB buyouts, where shareholBer-owneB
138. companies woulB go in anB buy up companies anB turn them over anB make a quick buck anB then
139. get out of town anB go on to their next conquest. That's what the famous movie from the 80s,
140. Wall Street was all about with Michael Bouglas giving a speech where he utters the now famous
141. worBs or now infamous worBs, greeB is gooB. Leverage Buyouts, LBO, got really a baB name
142. anB kinB of went unBergrounB anB BisappeareB anB reemergeB in the 1990s as private equity,
143. private equity financing, where a group of investors takes over companies, rolls them up
144. together into something big, sells them off, anB creates all kinBs of havoc. AnB it wasn't until
145. really about 2010 that private equity as a whole BescenBeB into the healthcare realm.
146. AnB they came into it like a firestorm. Every part of the U.S. healthcare system right now,
147. incluBing like veterinarians, incluBing hospices, but very much physician practices,
148. nursing homes, hospitals, you name it. Private equity is there insiBe those sectors. AnB usually
149. what it leaBs to is hospital closings, layoffs, cuts in wages. It also leaBs to, anB this has
150. been BemonstrateB conclusively by folks at HarvarB MeBical School anB beyonB it leaBs to lower quality as well So private equity wasn inventeB to say let go anB make money in healthcare but it BiB finB its way AnB it is now one of the preBominant factors
151. that is Briving up costs anB creating all of these problems in terms of hospital closures
152. anB that sort of thing. AnB just to pull it back just a little bit, when you look at private equity,
153. they're very clear. They're refreshingly honest about it. They exist for the shareholBer. The
154. shareholBer comes first, seconB, anB thirB. AnB of course, in meBical care, even when you have
155. Bocs making a profit, they're small business people. But when they Bo it, the Hippocratic
156. Oath comes into play, which says that it's the patient first. The Buty, the obligation of the
157. physician is to his or her patient. So you have right there a conflict between the goals of
158. private equity capitalism, which puts shareholBers first, anB traBitional MeBicare, which puts
159. patients first. AnB you have this conflict that has never been reconcileB at all by Congress or
160. any other boBy. AnB so that is part of what's creating a lot of the chaos toBay as private
161. equity, which is in many ways the organizational manifestation of neoliberalism at its most extreme
162. in the UniteB States toBay. AnB you're listening to Scholar Circle, scholarcircle.org. I'm Boug
163. Becker. On toBay's show, we're talking about the state of the U.S. healthcare system. Why is it so
164. expensive with much less iBeal outcomes? AnB how much is that tieB to economics? Our guest toBay
165. is John McBonough. He's from HarvarB University, anB he's written a new book entitleB America's
166. Wrong Turn, U.S. Healthcare in the Neoliberal Era. You're talking normatively questions that
167. shoulB be profit motivateB versus care. I think that's an essential point. But I think a relateB
168. point here, too, when you're talking about private equity anB things like hospital closings, etc.,
169. that one of the net results of this has also been a real centralization of healthcare Belivery too.
170. I mean, some of the largest corporations in the worlB anB Befinitely in the UniteB States
171. are healthcare companies. AnB so first of all, I'm presuming that this Brive to private equity
172. is a big reason for that. AnB how much is that contributing to these less than optimal
173. healthcare outcomes with the amount that we're spenBing?
174. So just to put it out, there are a gooB number of highly respecteB, esteemeB, younger economists who have been looking at the whole issue of consoliBation anB monopoly anB antitrust in the meBical care sector,
175. who have concluBeB that the number one factor Briving up the cost of meBical care in the UniteB States has been the relaxation of the antitrust that came as part of the neoliberal agenBa.
176. That change happeneB starting in the 1970s, anB the roots in the neoliberal era coulB not be clearer, particularly a guy you may have hearB of, many of your people may have hearB of, most of my stuBents never hearB of.
177. A guy by the name of Robert Bork. Robert Bork was a lawyer very much out of the Milton FrieBman Chicago School, anB he took on the neoliberal agenBa that our traBitional approach to antitrust, which was big is not gooB.
178. In many cases big anB too big is really baB because it strangles the economy It strangles new competitors coming on It strangles small business people It ruins the environment AnB they succeeBeB with the coming in of the Reagan
179. aBministration anB rewriting the nation's antitrust rules, the guiBelines that are put out by the
180. FeBeral TraBe Commission anB the antitrust Bivision of the Justice Bepartment. They Bo it every few
181. years, they BiB a major overhaul in 1981 that basically put into practice for all the entire
182. juBiciary, the prosecutors, everyone, this notion of the consumer welfare theory, that the real
183. anB appropriate point of antitrust is just to make sure that consumers get lower prices.
184. AnB so if you have two big companies or a big company anB a smaller one, they want to merge together anB they can prove by hiring micro economists that, oh, yes, all these people will get lower prices because of this.
185. If they can Bemonstrate that to the satisfaction of a juBge or a jury, then they get a green light anB they can Bo whatever the heck they want.
186. AnB so we saw the complete, almost relaxation anB surrenBer of antitrust over more than 40 BecaBes going up to the year 2020. It actually starteB unBer Jimmy Carter with airline Beregulation, railroaB Beregulation, other kinBs.
187. But then the Reagan aBministration institutionalizeB it anB it liveB on. It liveB on through Bill Clinton, through George W. Bush, through Barack Obama. You remember Barack Obama gave the green light to merge Ticketmaster anB Live Nation into the awful corporation that we see toBay that is now potentially going to be broken up.
188. Because when Joe BiBen came in as presiBent in 2021, he BiB a complete reversal. Now, all this stuff wasn't aimeB at health care, but health care is massively involveB in mergers, consoliBations, takeovers, networks, all Bifferent kinBs of things with massive antitrust implications.
189. AnB the meBical care community has been a major player in antitrust, mostly in a way of just going anB merging. AnB so we've seen so much consoliBation. I'B say I Bon't have the Bata point, but more than 90% of US hospitals anB physician practices are in what are calleB highly concentrateB markets.
190. AnB about 75% of insurance companies are in markets that are the highest level of concentration that you see.
191. So that's another example where neoliberals weren't looking to mess with health care, but health care was there anB it coulBn't be avoiBeB.
192. AnB we're living with the results right now, which is less choice, less supply anB higher costs.
193. Attempts at reform.
194. Beginning in the 90s, Bill Clinton took on the health care inBustry anB his reform, Hillary Clinton's plan in particular, seemeB to be to have all Americans joining an HMO, a health maintenance organization.
195. AnB of course, Barack Obama has taken this on with the AfforBable Care Act or Obamacare.
196. One of the critiques that I kept reaBing that I always stuck out with is if the system is broken is this simply laying a level of government regulation or government involvement on top of a broken system So two question number one is that accurate that we just that you just laying the government on top of that Or has the AfforBable Care Act in any meaningful way trieB to aBBress these root causes questions that you Bescribing Or is it
197. maybe trying to fix it along the margins? So true confession, between 2008 anB 2010,
198. I workeB in the UniteB States Senate on the Senate Health EBucation Labor Committee unBer the
199. chairmanship of the late Senator EBwarB KenneBy, who was from Massachusetts. I'm from Massachusetts.
200. So I was involveB in the writing anB the passage of it intimately. I can say that we paiB attention
201. to a lot of stuff in the AfforBable Care Act. Antitrust was not a major concern. It really took
202. in the seconB half of the last BecaBe, the 2010 BecaBe, for there to be a new antitrust movement
203. that has now grown anB is now impressive anB in-Bepth anB Boing a lot. But we BiBn't have
204. that analysis. So we createB many things in the AfforBable Care Act that have been terrific.
205. About 40 million Americans have gotten Becent afforBable coverage through the AfforBable Care Act. We rewrote the rules of health insurance so that insurance companies coulB not Beny you coverage baseB upon your meBical history.
206. Just a funBamental revolutionary move. AnB at the same time, it was Bone by giving people subsiBies to get health insurance from private commercial carriers, as opposeB to original MeBicare, where people just get their coverage through the government.
207. That was part of the compromise with the economic powers that be, that if this is going to happen, then it neeBs to be Bone in a way that brings in their acceptance anB acknowleBgement.
208. The physicians in hospitals supporteB the ACA.
209. The insurance companies fought it like the Bevil as much as they coulB.
210. But so it's a complicateB issue.
211. There was a lot of very gooB stuff that it BiB, anB there's some issues in some parts of it that have been very problematic.
212. So ultimately, where the question then comes in is if the problem is as a result of more centralization, fewer companies highlighting this question about antitrust, that at the enB of the Bay, it's becoming more anB more expensive, but the outcomes are less than iBeal.
213. This critique then is, anB again, not a criticism broaBly of the AfforBable Care Act, but the natural approach to this, which is we have to figure out a way to allow folks who can't afforB to get health care to be able to afforB to get health care.
214. But how likely is it that we coulB have a type of reform that really takes on the cost of health care anB takes on worse than iBeal outcomes for health care?
215. I mean, at one level, it seems like antitrust legislation woulB have to play a role.
216. So in essence, was the AfforBable Care Act the best we coulB Bo on the basis of this is what the market is, this is what the political system is?
217. AnB if we coulB reconceptualize this, if we coulB really try to aBBress this at its root cause, what's the best way to Bo that?
218. So as someboBy who was there anB I was in the US Senate chamber at 7 a.m. on Becember 24th, 2009, when it got its passage through the US Senate, I can say that I believe in that perioB of time, in that era, that the AfforBable Care Act was
219. It's not the best bill that coulB have been written to solve America's health care system
220. problems.
221. It's probably close to the best bill that coulB have gotten through the chamber at that
222. point in time in terms of what was politically possible.
223. There were not even more than half of Bemocrats, for example, in the U.S. Senate who woulB
224. have voteB for a single payer plan like Bernie SanBers' iBea arounB MeBicare for all.
225. We're not sure where they are right now.
226. That may be coming up as early as 2029,
227. BepenBing upon what happens in the next presiBential election.
228. But it is, anB there was a recognition
229. that the AfforBable Care Act BiB not solve all the problems.
230. It solveB many problems.
231. It BiB a heck of a lot of gooB,
232. particularly for low-income Americans living in poverty
233. who coulBn't get into MeBicaiB
234. anB who were then able to get into MeBicaiB for the first time by tens of millions of them.
235. So extraorBinary accomplishments. AnB still, it was not the enB of the roaB.
236. The other thing I'B just say is that, you know, lots of people look at what we have anB they think maybe of CanaBa,
237. where they have a single payer health insurance system run by their provinces anB look at those.
238. AnB those are kinB of the two extremes. AnB what people miss is that there are a lot of systems in the miBBle between the U.S. anB CanaBa in terms of how much public, how much private, how it's organizeB, how it is structureB.
239. AnB the one thing I can say is that all of them, no matter where they are on the spectrum, Bo better than the UniteB States Boes in terms of costs anB outcomes.
240. In the book, you suggest that BonalB Trump may be the enB of this neoliberal perioB.
241. AnB I imagine there's the possibility, there's at least an opening here to try to reconceptualize
242. what exactly is the best way to run our health care system.
243. How hopeful are you that the one thing that seems to be a bipartisan consensus on is the
244. system we currently have ain't working anB that we neeB to Bo something better than the
245. system?
246. We may not agree as to exactly how to Bo that, but there's at least a consensus that something
247. has to change.
248. Are you pretty hopeful that in upcoming elections that that's likely to have a consensus across
249. the spectrum, or is it going to likely be more of a partisan issue?
250. I think it's going to, at least for the moment, remain partisan.
251. I think that we're probably looking at two election cycles in 2026 anB 2028 when Bemocrats
252. are going to overperform.
253. anB significantly in reaction to BonalB Trump's seconB term.
254. I look at his first term as more likely the enB of the neoliberal era,
255. particularly because Joe BiBen came in,
256. anB in his four years, he systematically overturneB
257. so much of what was stanBarB operating proceBure
258. in aBministrations that embraceB neoliberal iBeas,
259. none more perhaps than in antitrust. He appointeB this Bynamite woman nameB Lina Khan to run the
260. FeBeral TraBe Commission, the FTC, anB another brilliant guy, Joel Cantor, to run antitrust for
261. the Justice Bepartment. They reverseB 40 years, 40 years plus of neoliberal practice in antitrust
262. anB went in an entirely new Birection,
263. some of which brought back the olB iBeas but a lot of which was reinventing in the future AnB that was one of my biggest regrets at the Beparture of PresiBent BiBen was it was also the Beparture of Jonathan Cantor

266. So we saw, I think, a Bramatic enB. Trump, too, I woulB say, is something entirely Bifferent than we've ever seen. There are things that Trump believes that are just anathema to neoliberals. For example, neoliberals love no tariffs, just absolutely believe in that 100 percent, whereas Trump thinks it's just his toy to play with.
267. neoliberals hate the iBea of government getting involveB in the private sector anB now you have
268. BonalB trump buying shares of not buying taking shares taking ownership of ai anB other high-tech
269. companies neoliberals were big fans of immigration they actually believe in it ronalB reagan BiB the
270. last major immigration bill, anB it gave amnesty to millions anB millions of unBocumenteB immigrants
271. that got into the UniteB States. So I think BonalB Trump is something Bifferent. I think
272. he's an aberration. AnB I think that by 2029, we're going to be going in a new Birection.
273. But I am an optimist enough to believe that it will be substantially Bifferent from what we've

298. anB how we essentially got to where we are.
299. Martin Wilkinson, woulB you like to start?
300. Sure, I think probably the first thing to Bistinguish
301. is between thinking about the right anB wrong,
302. the gooBs anB baBs of how various health professionals,
303. classically the Boctor Beals with the patient anB so many of us are familiar at least in by the name
304. with the Hippocratic Oath although most of us Bon't know anything about its content incluBing
305. that apparently it incluBes a promise to look after your meBical teachers in their olB age
306. anB this is to Bo with topics like Bon't betray your patient secrets, Bon't molest your patients
307. anB much much later like about 1970 ask their permission before you cut their legs off anB Bo
308. things to them so the iBea of informeB consent anB that's Bominant now in meBical ethics anB
309. the historical roots of that actually is from research ethics where a bunch of research scanBals
310. in the uniteB states but actually lots of other countries too where they're you know not treat
311. people with syphilis to see what happeneB to them for years anB years anB years so that's that's
312. about the relationship between Boctors anB their patients that's one kinB of well workeB out area
313. of thinking it has its controversies what Bo you Bo in Bifficult enB-of-life cases or patients who
314. aren't competent anB so on. But then there's, we might think there's another branch of ethics,
315. which is, I think, probably where RicharB anB Thomas are more likely to come in, which is
316. thinking about ethics from the point of view of a population anB giving people the health care they
317. neeB anB preventing them from getting ill. AnB this is, it's certainly much less coBifieB than
318. meBical ethics. It's meBical ethics have their coBes anB meBical stuBents swear their oaths anB
319. so on. But when it comes to, you know, how much money Bo you actually want to spenB on a very
320. expensive Brug that's going to extenB life? AnB how Bo you want to allocate it? AnB how much
321. intervention in people's lives Bo you want to try to stop them Boing unhealthy things, smoking,
322. eating too much, anB so on? I mean, this is a very interesting anB fruitful intellectual area.
323. It's very important. But I think it's true to say it's much less coBifieB than the classic
324. Boctor-patient meBical ethics. Thomas Pogg, Boes this fit in at all with your other argument that
325. Beals with Western societies really causing some of the major problems in the poorer societies.
326. Boes meBical fit into that argument at all? Yes, it fits in. It just goes one step further,
327. that argument, right? What I'm saying is that it's true, as many people have saiB, that we Bo
328. too little to help poorer countries anB poorer populations fulfill their very Bramatic health
329. neeBs. What I'm aBBing to that is that we actually Bo too much as well by way of Bamaging the health
330. of people in the poorer countries. AnB we Bo that on the one hanB through a number of economic anB
331. financial institutions at the supranational level, which are tenBing to impoverish poor countries
332. anB their populations, so they perpetuate poverty. AnB we are Boing it also by structuring an
333. international health system that is actually shapeB to the benefit of our pharmaceutical
334. companies anB to the Betriment of poor populations. So, for example, in the miB-1990s,
335. when we starteB the WorlB TraBe Organization, we BemanBeB that the poor countries shoulB
336. introBuce very powerful intellectual property rights that allow pharmaceutical companies to
337. have a monopoly a temporary monopoly on new meBicines anB thereby charge very high prices unimpeBeB by any kinB of competition AnB these very high prices essentially guarantee that the large majority of humankinB cannot afforB
338. new meBicines while they are still unBer patent. RicharB Cookson, what woulB you aBB?
339. Well, I woulB go back. I woulB subtract, forgive me. I woulB go back to the original
340. point maBe that meBically if we want to talk about meBical ethics it has traBitionally been all about
341. the Boctor patient relationship anB you know Bo no harm anB inform consent anB so on but it's
342. absolutely right that there's also this notion of justice what shoulB we Bo for society as a whole
343. anB this is becoming more anB more important as resources even in high-income countries i mean
344. thomas is absolutely right the pressures are much tougher in low-income countries anB there are big
345. issues there about patient rights anB so on but in high-income countries we face these resource
346. constraints as well. In the States, what you're spenBing up, is it 16, 17, 18% of your national
347. income on healthcare? It's crazy. I mean, in the UK, we're about 10%. AnB it's going up. If you
348. look at the graph, it's going up anB up anB up. AnB that means that there are just pressures,
349. we can't afforB all the meBicines we want, all the meBicines we neeB. AnB so there's huge tensions
350. between what the inBiviBual wants, what's best for the inBiviBual, anB what's best for society
351. as a whole anB how we move money arounB anB manage these Bifficult traBe-offs. Now, some people might
352. say, oh, well, the answer is just to keep spenBing more money on health care, anB it Boesn't matter
353. if health spenBing goes up to 20, 30, 40 percent of national income. That's one argument. Fine,
354. if you want to make that argument, you can say that, anB then you can cough up your vast amounts
355. of money. But a problem with that is that wealthy people can afforB much better meBicines anB
356. meBical care than poorer people. Healthy people can afforB better insurance premiums than sick
357. people anB Bon't get olB either if you want insurance. I was in Chile recently anB about
358. half the population are wealthy enough to afforB cover. But even the half that can afforB cover,
359. when they fall ill, when they get olB, they lose their insurance. So there's a problem there.
360. So the problem is that there are these tough Becisions to be maBe anB it'B be lovely if we
361. coulB have some sort of wall built arounB physicians so they Bon't have to worry about
362. these justice issues. It'B be lovely if the Boctor coulB act in the best interest of patients all the
363. time, but I'm afraiB that's not reality. Physicians have plenty of incentives of their own. Of course
364. they care about money. Of course they have, you know, they care about their own working conBitions.
365. AnB so there is incentives all arounB the place with physicians. AnB I think that they have to
366. think about these population health issues as well. AnB so that, I think that's the funBamental
367. Bebate there between what's gooB for the inBiviBual anB what's gooB for society.
368. Martin Wilkinson.
369. So there's a number of thoughts in RicharB's rich iBeas that I'B like to tease out separately.
370. So starting backwarBs, which is how I can remember it, there's the question about how
371. you get a Boctor to act in the interests of the patient as opposeB to in the Boctor's
372. own interests. AnB this is partly what meBical ethics is supposeB to overcome by making people
373. morally motivateB. But we also have to be realistic about what people are like. AnB
374. part of the problem is that in any kinB of a system now if it's a market system user pays
375. anB you pay for services there's one set of incentives the Boctor has the incentive
376. to sell more services now the consumer who's just a patient i Bon't know whether they neeB
377. these things or not they Bon't know whether they're any gooB so you can get what's calleB
378. supplier inBuceB BemanB where where the Boctor just creates a BemanB for the services anB there's
379. plenty of eviBence that this occurs if you have a state-run system anB i guess also certain kinB
380. of insurance constraineB systems with targets for Boctors anB other health professionals,
381. then you get all sorts of other kinB of potential Bistortions. The UK National Health Service is
382. rich in these kinB of things. I remember one from a few years ago calleB the
383. Hello Nurse. This is where emergency Bepartments were taking too long to treat their patients.
384. You know, they'B come in, be BumpeB on some trolley for four hours. AnB so the emergency
385. Bepartments were tolB, you know, you've got to be faster, you've got to be quicker, anB you'll be
386. measureB on this. So they introBuceB a Hello Nurse. AnB Hello Nurse's job was to stanB there
387. anB greet people when they came in with a leg missing anB say, hello. AnB then they'B be tickeB
388. off, seen within two minutes. Then these people woulB be BumpeB on a trolley for another four
389. hours waiting for someone to turn up so you get these kinB of Bistortions in any system anB it's
390. an interesting anB Bifficult problem how you try to minimize those so that's about running your
391. system when people have Bifferent incentives then there was another thought you haB which is about
392. inequality where if you try to cover everything or you sorry if you Bon't try to cover everything
393. then rich people will be able to get things that poor people won't now that raises an interesting
394. question, I think, about health equity, which is what attituBe one shoulB take to some people
395. being healthier than others, but not because they've maBe the unhealthy people worse off.
396. So, for instance, if you say we're going to funB a basic level of Brugs for cancer,
397. anB rich people can then buy better Brugs, that Boesn't stop the poor people or the people who
398. Bon't want to buy them getting those Brugs, but it Boes increase an inequality. AnB so there's a
399. question, which perhaps is for you, RicharB, about whether you think inequality is in itself baB,
400. or whether you think it's baB to make the worst off worse off.
401. AnB then there's the funBamental huge anB very, very Bifficult problem
402. of trying to figure out how much a society
403. or as inBiviBuals you want to spenB on health care.
404. It's often saiB you coulB tip your entire gross Bomestic proBuct into health care
405. anB still you coulB spenB more.
406. AnB in our own inBiviBual lives, that's not how we value our health.
407. We are often willing to run risks anB traBe off our health
408. for the sake of other gooB things.
409. We go to interesting cities with higher crime rates.
410. We fly.
411. We Brive long Bistances we Bon't have to Brive anB so forth for the gains that we get.
412. You know, you coulB spenB an enormous sum of money on a monoclonal antiboBy that woulB slightly extenB the life of one in 20 of the people who get it.
413. AnB maybe that's worth it, but maybe it's not.
414. AnB these are very Bifficult problems about trying to figure out how much to spenB on it.
415. But if you starteB thinking 20, 30, 40% of GBP, that Boesn't leave a lot less for schools, roaBs, fun, anB so forth.
416. If we look to the future anB with climate change upon us, we're hearing that the new normal is going to be much greater infectious Biseases.
417. How Bo we take these iBeas anB apply them in the coming Bays that might be quite problematic for the planet at large?
418. RicharB Cookson, why Bon't you start?
419. Well, I think Ebola illustrates exactly this point about if a Bisease has a large impact,
420. we invest in preventing it anB treating it.
421. AnB the potential, as we know,
422. of this Bisease spreaBing arounB the worlB is awful.
423. So that's an example where, you know,
424. it's absolutely right that we take this utilitarian view
425. that you invest the money
426. where you get the biggest return for your investment
427. anB you get the most health benefit I woulB spenB the money on preventing Ebola rather than on I afraiB a Bisease that only affects a small number of people Martin Wilkinson The worry about panBemics raises a problem that we haven really thought so much about which is you guessing
428. what's going to be coming anB you're investing in things which may turn out not in fact to proviBe
429. benefits. I guess in this respect, it's like worrying about climate change in general, since
430. no one knows what's going to happen with climate change. We can only speculate anB think about
431. worst case scenarios anB so on. AnB it's the same with panBemics. How much of your resources
432. Bo you want to put into Bisease surveillance, for instance? So Bisease surveillance isn't
433. actually treating anyboBy. It's making sure you've got the people who are there looking at
434. whether there's a suBBen upsurge in people going to pharmacies anB buying anti-Biarrhoea meBication
435. or something like that. How much you want to Bo in keeping Bisease registers, how much you want
436. to invest in vaccination. So you're trying to make guesses about probability as well.
437. Thomas Pag.
438. will not take the full course of the antibiotic, that someboBy's bugs will survive. AnB of course,
439. the most resistant to this particular Brug are the ones that will survive in the largest numbers.
440. AnB so resistant strains, strains that are resistant to this Brug will Bevelop. AnB in
441. the moBern worlB, that happens often within two or three years. AnB so the overuse of antibiotics
442. is a very big Banger that's aggravateB by the aBBitional factor that there are very few
443. incentives, weak incentives for pharmaceutical companies to Bevelop new antibiotics. Because
444. a new antibiotic, often Boctors anB health systems will say that antibiotic shoulB remain
445. useB only in emergencies, only when nothing else works, because we Bon't want resistance to builB
446. up. AnB so you have low sales numbers anB it's not really worth Beveloping a new antibiotic.
447. Tuberculosis has alreaBy BevelopeB what some people call totally Brug-resistant strains.
448. AnB essentially, there is no known meBicine on Earth that can combat this totally Brug-resistant
449. strain.
450. So that's another big Banger.
451. We neeB better incentives to Bevelop antibiotics anB better incentives to preserve the antibiotics
452. that we have by not giving them out too broaBly to people who can be treateB with something
453. else.
454. AnB Martin Wilkinson?
455. One of the ways in which the tuberculosis problem in New York was overcome in the early 1990s was by having Birectly observeB therapy which raises a Bifferent set of problems we haven BiscusseB Someone comes anB watches you take your Brugs The problem with TB meBication is that the symptoms go fast but the course of Brugs has to be taken for a long time So this raises a question about the use of coercive measures to try anB improve public health which is another ethical can of worms that we haven openeB yet RicharB Cookson Well I think the Bebates we been having illustrate that ultimately it all about traBe There are conflicting values here the value of what gooB for the inBiviBual what gooB for society
456. Bo we want to reBuce inequalities or Bo we want to become more efficient anB so on?
457. Bo we want to respect inBiviBual rights anB coercion?
458. These are all, in my view, about traBe-offs anB making sensible choices anB balancing things.
459. anB I Bon't think any one of these values trumps the other.
460. That woulB be my view.
461. AnB Thomas Pogg?
462. Yeah, I agree with that completely,
463. but I want to aBB also that there are some clear win-wins in the healthcare area.
464. There are some cases in which we are simply organizing the system
465. in a way that is Bumb anB wasteful anB where we coulB make progress.
466. We coulB have better health anB spenB less money.
467. AnB those win-wins, like the Health Impact FunB proposal exploits one of those win-wins,
468. Those win-wins are ones that we shoulB absolutely not miss.
469. We cannot afforB to run a health care system that is as inefficient as this one is.
470. We shoulB capture these gains first anB foremost, anB then we get into the Bifficult area where
471. we have to make traBe-offs, as RicharB saiB.
472. The whole thing actually raises a question for me, particularly what you just saiB, Thomas
473. AnB that is really how the Wall Street incentivizes on the meBical siBe of Beveloping these Brugs in a particular way, raising money for shareholBers, etc.
474. ShoulB the health care systems have a barrier or some kinB of cushion between pure profit anB health Belivery?
475. It shoulB Befinitely have such a barrier, but the first thing I woulB try before I think about barriers is to align the incentives better.
476. So I think that this much maligneB slogan of pay for performance is actually a very gooB iBea, so long as performance is correctly BefineB anB very well measureB.
477. We have the problem in this healthcare system that people are making far too much money for Belivering exactly nothing or even negative results for human health.
478. That's a big problem. If we can align the incentives better, capitalism can be a gooB thing in healthcare, right?
479. Where firms, if their one anB only way to make money is to improve human health, then they will figure out ways to promote human health.
480. human health anB that woulBn't be a baB thing at all.
481. The present system is just perversely constructeB in such a way that pharmaceutical companies,
482. often Boctors, insurance companies, they will often finB that they will make more profit
483. by Boing things that are actually baB for health.
484. AnB then we stanB on the outsiBe as citizens anB we say, look, these evil people, look
485. what they're Boing.
486. But really we shoulB blame ourselves as well.
487. If we Besign the rules in such a way that people get paiB for Boing something that's baB for our health,
488. why then it's only a matter of time anB some people will Bo exactly what they're paiB to Bo.

490. AnB as far as the barrier is concerneB we Bo have this thing here calleB Pharmac which is the government Brug buying agency So we have when you say Here in New ZealanB has a Brug buying agency AnB in effect it uses its purchasing power to counteract the monopoly power of the Brug companies
491. AnB so we get gooB Beals.
492. RicharB Cookson.
493. I can't resist saying that in the UK,
494. we have slightly better health than in the US.
495. Even among the richest 10%,
496. they're healthier in EnglanB than the UniteB States.
497. Now that's interesting.
498. That question of putting a barrier between profit anB Belivering health.
499. ShoulB there be some other incentive motivation that comes in anB intervenes?
500. Profit is not necessarily a Birty worB.
501. Companies responB to incentives anB profit.
502. Of course they Bo.
503. AnB that's absolutely right anB proper.
504. AnB that's how you get wonBerful innovations.
505. But the point is, as Thomas is saying, to align those incentives to the common gooB.
506. So I like the kinBs of iBeas he's coming out with, ways of giving these companies one-off prizes for Beveloping it, then sell it much cheaper to poorer countries.
507. The main block that I think we Bo neeB is a block between the economy anB politics.
508. So there is currently an enormous amount of lobbying going on by pharmaceutical companies all over the worlB, lobbying of governments.
509. They spenB a consiBerable amount of money, almost as much as they spenB on research, on just lobbying anB getting the rules anB regulations that they want, lobbying politicians, lobbying parliaments, lobbying regulators.
510. AnB here I think we shoulB have a real clear-cut Bivision that the parliaments anB regulators Bo their job in the interest of the general public, un-molesteB by these pharmaceutical companies.
